Intake
Project Extended Arms Intake:
If you are HIV positive and are in need of services, call Welcome House, Inc at 214-421-3948 to schedule an intake with the Client Advocate. Be prepared to provide the following information:
- A letter of diagnosis.
- Photo Identification (examples: photo ID, TX driver’s license, etc).
- Social Security Card.
- Proof of Income for the current year (examples: check stub, award letter from SSDI, copy of paycheck, etc).
- Proof of residency (examples: copy of lease agreement, utility bill with name and address, correct driver’s license, bank deposit slip, etc.
Intakes are typically scheduled within three to five business days, Monday through Friday from 10am until 3pm.
Operation Renew Intake:
Welcome! If you are in need of transitional housing and recovery support services, call Welcome House, Inc. at 214-887-0696 to schedule an intake with a Client Advocate. Be prepared to provide the following information:
- Photo Identification (examples: photo ID, TX driver’s license, etc).
- Social Security Card.
- Pay a one time administrative fee of $150.00 and the weekly fee is $135.00 ($540.00 monthly).
Intakes are scheduled from Monday through Friday from 9am-4pm (exceptions can be made).
